Insurance companies may require us to have prior authorization or follow a specific referral process in order for our services to be covered under a patient's insurance plan.

  

Most insurance companies require a co-pay or a 20% coinsurance for each date of service to be paid by the patient.

 

When scheduling your initial appointment, we will need your insurance information.

We currently accept most major insurances; however we do not accept Medicaid.

Family Health Plus, Child Health Plus and HealthyNY are accepted with a referral.

 

We also accept most Workers Compensation and No Fault insurances.  In New York State a referral from a physician is required for No Fault and Worker's Compensation.  Prior authorization by the insurance carrier is required before treatment.
